To do so, just copy the information below between the swords, paste it in an e-mail to me, and fill out the information requested.  I require your name to publish, however, if you do not want others contacting you just let me know and I will not publish your e-mail address. I am working on a form for ease of submissions.

While I will consider others, this is a service to Viet Nam Vets and that is primarily who I designed it for.
